Using an emulator is great and all but is it possible with something like Free McBoot to run mods on a console?

It is, actually! In Persona 3 FES, I've managed to get both CVM-based mods (example: music mods) and SLUS-based mods (party control, widescreen patch) working on real hardware, though the process isn't nearly as streamlined as it is for playing on an emulator.

It also requires more than just Free MCBoot, you'll also need either a bunch of blank DVDs that you can burn and an ESR loader, or if you have a "phat" model PS2, you can also try the official Sony PS2 Network/HDD add-on module for loading ISOs more like an emulator.

You could theoretically use a USB flash drive, but it is NOT RECOMMENDED, this method requires a perfectly defragmented USB drive after copying the ISO and lags severely on FMV cutscenes because the PS2's USB ports are super slow due to how old the system is. There's also a way to boot games stored on your PC's hard drive by creating a SMB network share and directly connecting your PS2's network adapter to your PC via an ethernet cable, but it requires the use of an insecure protocol that could introduce severe security risks to your computer.

Whatever method you choose, it's highly recommended that you test any mods on an emulator before using them on hardware, otherwise you risk wasting a perfectly good blank DVD or spending 20 minutes copying an ISO to your PS2's HDD.
I can be your best friend or your worst enemy. It all depends on you.
